Your CCAA vaccination certificate (vaccination history)

What you need to have to hand

1. Cl@ve PIN or Permanente, certificado digital or DNIe. Without one you’ll have to go in person.
2. Tarjeta sanitaria or your CIP/CIPA/TSI code for the community.
3. Carné de vacunación or reports if you were vaccinated privately or in another CCAA (date, batch and centre).
4. If you request it for a minor: your DNI, libro de familia or guardianship document and, if the website won’t allow it, digital representation activated.

Documentation

1. Identification: DNI/NIE (in person). Online you authenticate with Cl@ve/certificado.
2. Health data: tarjeta sanitaria or your personal health code for your CCAA.
3. Proof for external vaccines: reports/carné signed by the centre, with dates and batch numbers.
4. Authorization for representation for minors/wards if the portal requires it.

Step by step

1. Access your health portal: log in with Cl@ve/certificado to your CCAA’s health folder (e.g.: Cataluña https://lamevasalut.gencat.cat, Madrid https://www.comunidad.madrid/servicios/salud/tarjeta-sanitaria-virtual, Andalucía https://www.sspa.juntadeandalucia.es/servicioandaluzdesalud/ciudadania/clicsalud, Galicia https://esaude.sergas.gal, Euskadi https://www.osakidetza.euskadi.eus).
2. Find “Vacunación/Historial vacunal”: it’s usually in your clinical history or ‘Mis informes’. Click “Certificado” or “Descargar”.
3. Set up the certificate: choose language if available and whether you want all vaccines or a specific one. Confirm.
4. Download the PDF with CSV/QR. Don’t edit it. Keep it safe; many employers/schools verify online.
5. Missing vaccines? Use the portal’s aportación de documentación option (if it has one) and upload carné/report. If your portal lacks that, go to the next step.
6. Plan B in person: book a cita en tu centro de salud (administration) and take DNI and proof of external vaccines. They’ll print it and, if needed, record missing data.

Time-saving tricks

1. Cl@ve Permanente fails less than PIN on these portals. Activate it if you can.
2. Scan the carné well: a single page with clear dates and batch numbers speeds up registration.
3. Ask for the full certificate, not one per vaccine. You avoid back-and-forth if they later ask for ‘all’ vaccines.
4. If the portal kicks you out, try another browser. Some sites work poorly on older phones.
5. Save the CSV (code) in a notes app. If the PDF is rejected, showing the CSV helps verification.

Common mistakes that will mess up the process

1. Assuming everything appears automatically: vaccines from private providers or another CCAA won’t show until someone uploads proof.
2. Editing the PDF to ‘remove data’ or translate it: that invalidates the CSV/QR and they can reject it.
3. Showing up without ID for minors: without libro de familia/tutela they won’t give it.
4. Trusting it will be instant: if they must add old vaccines, it can take days. Don’t leave it to the last minute.
5. No digital access: without Cl@ve/certificado you can’t download. If you’re in a rush, go in person.
