- 1.Check your empadronamiento and download a recent volante from your ayuntamiento's sede (you absolutely need it).
- 2.If you don't have NUSS/NAF, go to importass.seg-social.es and get your Social Security number before requesting the TSI.
- 3.If you don't appear as entitled to healthcare, first process “Asistencia sanitaria. Reconocimiento del derecho” on sede.seg-social.gob.es (attach DNI/NIE and padrón).
- 4.Submit the application via the Registro Electrónico Común (REC) at rec.redsara.es addressed to the Servicio de Salud of your CCAA: subject “Solicitud de Tarjeta Sanitaria Individual (alta/traslado/cambio/beneficiario)”, and attach all PDFs.
- 5.If you are EU with S1 or come from another CCAA, attach the S1 and/or your old TSI; state your preferred health centre and municipality in Observaciones.
- 6.Watch notifications on carpetaciudadana.gob.es and notificaciones.060.es; when they confirm your registration they assign you a doctor and the physical card arrives by post (meanwhile, ask your health centre for a proof of assignment).
