Procedure 12: Additional health card (add a beneficiary or request a duplicate tarjeta sanitaria)

What to have ready
  1. Cl@ve, certificado digital or DNIe. Without one you’ll need a cita presencial.
  2. Padrón reciente (less than 3 months). If you’ve just moved, wait until the padrón is updated.
  3. Documento del INSS that recognises your derecho a asistencia (asegurado/beneficiario). If you don’t have it, sort it first.
  4. Proof of relationship: Libro de familia, matrimonio or registro de pareja de hecho; for minors, partida de nacimiento.
  5. PDF scans of everything, legible and under the sede limits (many cap files at 5 MB).
Documentation
  • DNI/NIE of the holder and the beneficiary.
  • Certificado de empadronamiento from the municipality of your CCAA (recent).
  • Resolución del INSS recognising the holder’s derecho a asistencia (or equivalent status).
  • Libro de familia/registro de pareja de hecho; for students >26, matrícula; for EU cases, S1 if it applies.
  • Tarjeta sanitaria of the holder or its number.
  • Autorización firmada if another person files + their DNI.
Step by step
  1. Access the sede of your servicio de salud and log in with Cl@ve/certificado.
  2. Search “Tarjeta sanitaria / Alta de beneficiario / Duplicado” and open the request.
  3. Fill in the details for the holder and the beneficiary. Pick your centro de salud and delivery address.
  4. Attach the PDFs: padrón, INSS, Libro de familia, etc. If the sede errors on size, compress the files.
  5. Sign and register. Download the justificante de registro (it saves you from headaches).
  6. If you lack INSS recognition, go to the INSS sede electrónica and request it; then come back to complete the alta.
  7. Track the expediente via Carpeta Ciudadana. They’ll alert you by SMS/email. The card comes by post or via your CCAA’s health app.
Time-saving tricks
  • Upload everything as one PDF per section if the sede accepts it; validations fail less.
  • Get the padrón online from your ayuntamiento before you start so you don’t get blocked mid-way.
  • File in the holder’s name with their Cl@ve: avoids authorizaciones and extra trips.
  • If it’s urgent, ask your centro de salud to activate a CIP provisional using the justificante.
  • Watch appointment releases: Mondays at first hour often free up slots.
Common mistakes that mess up the trámite
  1. Expired padrón: older than 3 months and they stop it.
  2. Holder not recognised by INSS: the CCAA can’t invent coverage; without aseguramiento they reject it.
  3. Partner without registro or proof of convivencia: no proof, no beneficiary.
  4. Files too large or unreadable: rejected for “documentación ilegible”.
  5. Mismatched data (surname, address, birth date): ask for updates in padrón/INSS before retrying.

  2. 2.Find the procedure called “Tarjeta sanitaria / Alta de beneficiario / Duplicado” and open the form. It’s usually under tarjeta sanitaria or población.
  3. 3.Fill in the holder's details and the person to include. Give the delivery address and select the assigned centro de salud if it asks.
  4. 4.Attach scanned documents in PDF (padrón, libro de familia, INSS resolution, etc.). Watch file size: many sedes reject files >5 MB.
  5. 5.Sign and register the application electronically. Save the justificante de registro and the expediente number.
  6. 6.If you lack the reconocimiento del derecho a asistencia, do that first at the INSS (Reconocimiento de derecho a asistencia sanitaria) and then return to the regional trámite: https://sede.seg-social.gob.es/wps/portal/sede/sede/Ciudadanos/AsistenciaSanitaria/27334
  7. 7.You’ll get notified via Carpeta Ciudadana or SMS when it’s ready (check https://carpetaciudadana.gob.es). The card arrives by post or appears in your CCAA’s health app.
  • DNI/NIE of the holder and of the person to include (if they have no NIE, passport + NIE in trámite usually does not count)
  • Certificado de empadronamiento reciente (less than 3 months), for the holder and/or the beneficiary as required by your CCAA
  • Document from the INSS that proves the holder's right to asistencia (reconocimiento de asegurado/beneficiario or alta in the Seguridad Social)
  • Libro de familia or certificado de matrimonio/registro de pareja de hecho; for minors, partida de nacimiento
  • Tarjeta sanitaria actual of the holder (SIP/TIS/CIP autonómico) or its number
  • Signed autorización if someone else files it + their DNI/NIE
  • If applicable: S1 (pensioners from the EU), tutela/guarda resolution, or certificado de estudiante if the beneficiary is over 26
