Procedure 2: Additional health card
Health card changes have waiting lists. We alert you when a slot opens.
Request an additional tarjeta sanitaria for a family member under your care (beneficiary) or a duplicate within your autonomous community. Useful if you want to add your children/partner to your card or need another card for the same household.

- 1.Check and download your Certificado de derecho a asistencia sanitaria at Sede de la Seguridad Social: https://sede.seg-social.gob.es (Ciudadanos > Asistencia sanitaria).
- 2.Book an appointment or start the Tarjeta Sanitaria Individual request on your CCAA health portal (for example: Madrid https://www.comunidad.madrid/servicios/salud/tarjeta-sanitaria-individual or Comunitat Valenciana https://www.gva.es/es/inicio/procedimientos?id_proc=17358).
- 3.Scan the documents to PDF/JPG (DNI/NIE, empadronamiento, libro de familia, holder’s tarjeta sanitaria). Make sure they are legible and glare-free.
- 4.If doing it online, log in with Cl@ve/certificado, fill the form and attach the files. Sign and submit via registro electrónico.
- 5.If doing it in person, print the form if your CCAA requires it, sign and go to the health centre with prior cita previa and all originals and copies.
- 6.Check the status in your autonomous health portal/carpeta de salud and keep the provisional receipt. The physical card arrives by post; if it doesn’t arrive in 4 weeks, complain through the same portal or at your health centre.
