Procedure 5 – Additional healthcare: add a beneficiary to your health card (CCAA)
Health card changes have waiting lists. We alert you when a slot opens.
So your partner or kids are registered as beneficiaries of your public healthcare. First the INSS recognises the right, then your CCAA issues the health card.

- 1.Go to the Sede de la Seguridad Social (sede.seg-social.gob.es) > Ciudadanos > Asistencia sanitaria > “Reconocimiento del derecho a la asistencia sanitaria”. ID yourself with Cl@ve or certificado digital.
- 2.Fill in the application as Titular and indicate you want to add a Beneficiario. Enter their details and upload PDFs: empadronamiento colectivo, identity documents and Libro de familia or Registro Civil certificate.
- 3.Send it and save the receipt. Check “Mis notificaciones” on the Sede. The INSS resolution usually takes 5–15 working days.
- 4.With a favourable resolution, go to your CCAA’s Servicio de Salud portal and request the Tarjeta Sanitaria Individual (TSI) for the beneficiary. If your CCAA accepts online, upload everything there; if not, take it to your centro de salud.
- 5.Attach the INSS resolution, the empadronamiento and identity documents. Select the centro de salud/médico de familia if the system asks.
- 6.They confirm the beneficiary registration. The TSI can take a few days to arrive by post or they activate it digitally in your CCAA app.
