- Fill the modelo. Be specific. If your CCAA gives templates with options, tick what you want and what you don’t. If you name a representative, include full details.
- Choose validation: a) Witnesses (two, no financial interest, with DNI); or b) Notary (you pay their fee, but you avoid witnesses and get immediate validation).
- Book a cita at a punto de registro (hospital/centro de salud/registro autonómico). Use the link on your CCAA page and reserve. Heads-up: some places only offer morning slots and they go fast.
- Attend the cita with everything: document, DNI, tarjeta sanitaria and witnesses or the escritura notarial. They check IDs, read the content and inscribe it. They give you a resguardo with CSV/registration number.
- Check the inscription. After a week, log in to your health portal and see if it’s recorded. If not, call the punto de registro and give them the receipt number.
Time-savers and tricks
- Sign every page if the modelo requires it. Many get bounced for missing signatures.
- If you go with witnesses, make sure they’re identified and not heirs. You avoid rejections.
- If your CCAA allows online processing with Cl@ve, have your certificado digital ready and the PDF under 5 MB. Many sites enforce that limit.
- Bring two copies: one for the registry, one for you. Upload your copy to your Carpeta de Salud.
Common mistakes to avoid
- Wrong modelo (from another CCAA): they’ll return it. Use the one for your community.
- Bad witnesses (family beneficiaries or no DNI): they won’t register it.
- Ambiguous document: if it’s unclear, doctors default to standard treatment. Be explicit.
- Not checking the inscription: if it’s not in your record, emergency staff won’t know. Check and complain if needed.
- Thinking a copy on your phone is enough: without registration it might have no effect. Register and keep the receipt.

- 2.Download the official modelo de tu CCAA and fill it: your details, representative (if any), and your instructions (resuscitation, treatments, care, donation, etc.). Sign every page if the form asks.
- 3.Decide validation method: a) go with two adult witnesses with their DNI/NIE; or b) do it before a notary. If you choose notary, bring the escritura on registration day and you avoid witnesses.
- 4.Book an appointment (cita previa) at a registration point of your CCAA (usually hospitals/centres de salud). Examples: in Madrid the page above lists points and contacts; in Andalucía you can book from ClicSalud+ (https://www.clicsaludplus.es/); in Catalunya the Canal Salut page explains where to register.
- 5.Attend the appointment with your DNI, tarjeta sanitaria, signed document and witnesses (or the notarial deed). They verify identities, read the document and register it. They give you a receipt with CSV or registration number.
- 6.Keep the receipt and upload the PDF to your Carpeta de Salud if your CCAA allows it. Check after a week that it appears in your medical record (if not, contact the registration point).

- If your CCAA asks for it: recent certificado de empadronamiento (typical if they can’t find you in their database)
If it’s not registered, the hospital might not apply it
If you only keep the document in a drawer, emergency staff might not see it. Register it in the registro autonómico so it shows in your medical record. Carry a copy until it’s on file.
Witnesses must bring their ID and not be heirs
If you sign with witnesses and one doesn’t bring DNI or is a direct heir, they’ll reject the process. Bring properly identified witnesses who aren’t beneficiaries.
Changing or revoking it is free
You can modify or cancel it any time, but you must register the new document (another appointment or a notary). The last registered version is the valid one.
Can I do it online with Cl@ve and skip witnesses?
Depends on your CCAA. Some require physical presence or a notary; others allow electronic processing with advanced electronic signature and later verification. Check your community’s page and if you don’t see an online option, you’ll need an appointment.
Is it valid in another community if something happens while I’m away?
Yes. Autonomic registers communicate with the national register and any public hospital can consult it. Still, carry a copy until you see it in your medical record.
Do I always need a notary?
No. In almost all CCAAs you can choose two witnesses or appear before staff at the registro. Notary is an alternative if you want to avoid witnesses.
Can I name a family member as my representative?
Yes. Pick someone you trust who understands your wishes. Avoid anyone with a conflict of interest. Note their DNI and phone; update it if you change your mind.
How long until it’s added to my medical record?
Usually 1–4 weeks from registration. If it’s not there after that, contact the registration point that stamped your receipt and give them the registration number or CSV.
Can they deny me care because I have this done?
No. It’s your informed will and has legal backing. In borderline cases they may consult an Ethics Committee, but your document guides decisions when you can’t decide.
