Procedure 7: Additional Healthcare

Register your instrucciones previas (voluntades anticipadas) with the health service of your CCAA so they appear in your medical record. Use it to record which treatments you accept or refuse when you can’t decide.

- 1.Prepare the instrucciones previas document: either sign it before a notary and get the escritura, or sign it with 2–3 witnesses according to your CCAA rules (witnesses must not be your beneficiaries or the healthcare staff treating you).
- 2.Go to the website of your CCAA and find “Registro de instrucciones previas/voluntades anticipadas” (examples: Madrid: https://www.comunidad.madrid/servicios/salud/registro-instrucciones-previas; Andalucía: https://www.juntadeandalucia.es/organismos/saludyfamilias/areas/ciudadania/garantias/paginas/instrucciones-previas.html; Cataluña: https://salut.gencat.cat/ca/temes/consentiment/voluntats-anticipades/).
- 3.Start the online application using Cl@ve/certificado and fill in your details. Attach PDFs: DNI, tarjeta sanitaria, the signed instrucciones previas document and, if applicable, the escritura notarial or the DNI scans of the witnesses.
- 4.Electronically sign the application (use AutoFirma if the sede requests it) and submit. Save the electronic receipt and the expediente number.
- 5.Check notifications in your Carpeta Ciudadana (https://carpetaciudadana.gob.es) or on your CCAA sede in case they ask you to correct something (they usually give 10 días hábiles).
- 6.When they register it, you’ll get a resolution. Check that it appears in your historia clínica or in your CCAA health portal. If it doesn’t, write to the registro unit listed in the resolution.
