Procedure 7: Additional Healthcare

What to have ready

1. DNI/NIE and tarjeta sanitaria of your CCAA.
2. Documento de instrucciones previas/voluntades anticipadas already prepared.
3. Witnesses (2–3 depending on CCAA) with their DNI or escritura notarial if you went to a notary.
4. Cl@ve, certificado digital or DNIe to sign online.
5. A browser that works with the sede (Chrome/Edge usually works better; sometimes AutoFirma is required).

Documentation

• Documento de voluntades anticipadas in the modelo de tu CCAA, signed.
• DNI/NIE of the grantor and tarjeta sanitaria.
• If witnesses: signed statement with names, DNI and signatures of the witnesses + scanned DNI of the witnesses.
• If notary: copia simple of the escritura notarial.
• Consent for data processing (usually included in the model).
• Any medical report you want to attach (optional; some CCAA accept it as a complement).

Step by step

1. Choose the route: witnesses or notary. Witnesses save money; notary saves doubts about validity. Don’t mix methods.
2. Fill the model of your CCAA. Make sure to clearly mark treatments you accept/refuse and who you name as representative if you want to.
3. Access the sede of your CCAA and go to “Registro de instrucciones previas/voluntades anticipadas”. Examples:
   • Madrid: https://www.comunidad.madrid/servicios/salud/registro-instrucciones-previas
   • Andalucía: https://www.juntadeandalucia.es/organismos/saludyfamilias/areas/ciudadania/garantias/paginas/instrucciones-previas.html
   • Cataluña: https://salut.gencat.cat/ca/temes/consentiment/voluntats-anticipades/
4. Start the online application with Cl@ve or certificado. Complete your details and attach the PDFs: DNI, tarjeta sanitaria and the document (and escritura or DNIs of witnesses as applicable).
5. Sign and register. If it asks for AutoFirma, open it when prompted. Save the resguardo del registro and the número de expediente.
6. Watch for notifications on the sede or at https://carpetaciudadana.gob.es. If they ask for a subsanación, you usually have 10 días hábiles.
7. Confirm the registration when the resolution arrives. Log into your portal de salud and verify it appears. If not, contact the unit listed in the resolution.

Time-savers

• Single PDF per document. Clear and readable. If the file is too big, compress it without ruining the text.
• If you use witnesses, make sure they are not your beneficiaries nor healthcare staff. You’ll avoid a pointless subsanación.
• Keep AutoFirma and Java up to date. Many odd errors come from outdated Java. If it fails, try a different browser.
• Name a representative in the document if you want someone to move things for you when you can’t.
• Keep a PDF copy of your voluntades on your phone. Interoperability between CCAA is not always perfect.

Typical mistakes that will mess up the process

• Mixing routes (uploading escritura notarial and a witnesses sheet at the same time). They’ll stop it.
• Invalid witnesses (beneficiaries, minors, no DNI). Guaranteed denial or subsanación.
• Bad electronic signatures (protected PDF, unrecognised signature). Use AutoFirma and sign where the form asks.
• Not checking notifications and missing the subsanación deadline. They’ll archive the expediente.
• Using another CCAA’s model. It looks the same but it isn’t. Each comunidad has its own format and ventanilla.
