This is to get a doctor/nurse assigned in the region where you’re temporarily based without losing your home centre. Handy if you move for studies, work or care for a few weeks/months and need a temporary tarjeta sanitaria there.

Step by step
- Find the procedure on your Servicio de Salud website. Log in with Cl@ve and locate “Desplazamiento temporal/Alta de desplazado”. If your CCAA has no online option, skip to step 2.
- Book a cita at administración of the health centre in your temporary area. Reason: trámites de tarjeta sanitaria/administración.
- Fill the application with start and end dates, and temporary address. Attach DNI/NIE, TSI and proof of stay. Check the PDF size (many sites choke if you upload +5 MB).
- Submit or sign. Online, sign with Cl@ve and save the receipt. In person, they’ll stamp a copy and give you the resguardo.
- Wait for assignment. You’ll get SMS/email and see it in your Carpeta/Portal de Salud. Some CCAA activate a virtual card in the app; others issue a temporary physical card.
- Collect or activate. If physical, go to the centre or wait for post. If virtual, it should be ready in 24–72 h.
Time-saving tips
- Realistic dates: don’t claim 1 year if you’re there one semester; they may deny or limit it.
- Fresh proof: padrón issued within 3 months if requested; clearly signed contracts.
- Browser: many sedes work better on Chrome. Disable blockers when uploading files.
- Cl@ve ready: if you use Cl@ve PIN, generate the PIN just before logging in; it expires fast.
- Peak times: September and January are a pain; book the appointment as soon as you have an address.
- Don’t unregister at home: this is an additional registration; your home doctor stays.
Common mistakes that can derail the process
- Not proving right to healthcare: without TSI or INSS resolution they limit you to emergencies and may bill non-urgent care.
- Weak proof of stay: an email from a friend won’t do; provide a contract, matrícula or company certificate.
- Inconsistent dates: if you request 6 months but upload a 4-month matrícula, they’ll shorten it.
- Huge or blurry files: the sede chokes on 20 MB PDFs; scan cleanly and compress.
- Letting the desplazamiento expire: if you don’t renew, you lose the assigned doctor there just when you need them.
- 1.Log in with Cl@ve or certificate on your Servicio de Salud portal and search “Desplazamiento temporal / Alta de desplazado”. If your CCAA doesn’t allow it online, go for a presencial appointment.
- 2.Book a cita previa at the health centre in the area where you’ll live via your Servicio de Salud website (reason: ‘Administración’ or ‘Trámites tarjeta sanitaria’).
- 3.Fill the desplazado application indicating start/end dates and temporary address. Attach DNI/NIE, your home TSI and the proof of stay (rental, studies, work).
- 4.If you go in person, bring originals and copies. Hand the application to administración at the centre; they’ll give you a receipt (resguardo).
- 5.Wait for the resolution: they assign you a local doctor/nurse. They’ll notify you by SMS/email and you’ll see it in your Carpeta/Portal de Salud of the CCAA.
- 6.If they issue a temporary physical card, pick it up at the centre or they’ll post it to you. If it’s virtual, it appears in the CCAA app within 24–72 h.
- DNI/NIE or valid passport
- Tarjeta Sanitaria Individual (TSI) from your home CCAA or INSS resolution proving the right to healthcare
- Número de la Seguridad Social (NAF/NUSS) or S1 document if you’re a beneficiary with rights in Spain
- Proof of temporary stay: rental contract, enrolment/registration for studies, employment contract or company certificate
- Certificado/volante de empadronamiento if required (some CCAA ask it if you stay more than X days; usually requested with issue date <3 months)
- Application form for alta como desplazado from the Servicio de Salud of the destination CCAA (online or at your health centre)
Limited duration
The alta de desplazado is usually granted for 30–90 days and is renewable. Each CCAA has its own deadlines; note the end date because, if it expires, they can stop assigning you a doctor.
Right to healthcare
If you can’t prove the right to healthcare (registration with INSS, beneficiary status or S1), you’ll only be treated for emergencies and they may bill non-urgent care. Keep your TSI or INSS resolution handy.
Empadronamiento and proofs
Some CCAA ask for empadronamiento for long stays. If you don’t have it, provide a rental contract or enrolment. Watch out in September/January: appointments get snapped up.
Do I lose my doctor in my home CCAA?
No. They assign you an additional doctor in the CCAA where you’re temporarily based, but your original doctor stays. When the desplazamiento ends, you keep your usual centre.
How long does the alta de desplazado last?
Depends on the CCAA, but usually 30–90 days. If you stay longer, renew it with another proof of stay. Do it before it expires so you don’t get left without care.
Do I need to register on the padrón for this?
Not always. If you don’t register, they usually accept a rental contract, enrolment or work contract. For long stays some CCAA do require padrón.
I’m a temporary student, does this work for me?
Yes. With your matrícula and TSI they’ll process the desplazamiento and assign you a doctor near the campus. Upload the matrícula as proof and set the course end date.
What if I get sick before it’s activated?
Emergencies will treat you regardless. For GP appointments and prescriptions wait for activation or try to see your home centre if you can.
Can this be done 100% online?
In several CCAA yes, with Cl@ve or certificate. Others force you to visit administración at the centre. Check your Servicio de Salud portal and, if it’s not there, book a cita.
