What you need to have ready

1. Cl@ve o certificado digital operational. If you don’t have one, forget the online route and go to the health centre.
2. Volante de empadronamiento recent. Get it first and you avoid a block.
3. Resolución de derecho a asistencia from the INSS if you don’t appear in their database (titular and/or beneficiario).
4. Documentos de vínculo (libro de familia, certificado del Registro Civil, pareja de hecho… depending on the case).
5. Scans in PDF clear and small (DNI/NIE, TIE, passport if needed).

Documentation

• DNI/NIE of the holder and each beneficiary.
• NAF (Social Security number) of the holder and, if they have it, of the beneficiaries.
• Empadronamiento individual or collective (<3 months).
• Resolución del INSS recognising the right to healthcare if it’s not recorded automatically.
• Libro de familia/certificado de vínculo. For minors or spouses/pareja de hecho.
• Extranjeros: TIE o certificado UE and passport.
• Tarjeta sanitaria of the holder (photo) if they ask to link beneficiaries.

Step by step

1. Secure the healthcare right. Log into the Seguridad Social with Cl@ve at https://sede.seg-social.gob.es/wps/portal/sede/sede/Ciudadanos/asistenciasanitaria/11139 and request recognition if needed. Upload padrón and vínculo. Keep the resolución.
2. Go to your CCAA portal. Search “Tarjeta Sanitaria Individual (alta/modificación)”. Examples: Madrid (https://sede.comunidad.madrid/ayudas-becas-subvenciones/tarjeta-sanitaria-individual), Catalunya (https://tramits.gencat.cat/es/tramits/tramite/2529-Tarjeta-sanitaria), Andalucía (https://www.clicsalud.es).
3. Fill “Añadir beneficiario”. Enter data exactly as in DNI/NIE and NAF. Attach empadronamiento, documento de vínculo and, if requested, the resolución del INSS. For foreigners, TIE/Certificado UE and passport.
4. Sign and register with Cl@ve or certificado digital. Download the justificante and the expediente number. If the portal offers it, enable SMS/email alerts.
5. Follow the expediente in your CCAA carpeta ciudadana or at https://carpetaciudadana.gob.es. Once approved you’ll see the assigned CIP/TSI for the beneficiary and instructions on whether they send the card or you collect it at the health centre.

Tricks that save you time

• Do the INSS bit first. If you don’t appear with the right, the CCAA will bounce the request. Better 10 minutes now than 2 weeks later.
• Small PDFs: scan at 150–200 dpi, black & white if acceptable. Many portals choke on files >10 MB.
• Browser: use Chrome or Edge. Signing often fails on Safari/Firefox.
• Matching names: ensure padrón, libro de familia and DNI show the same name. If diacritics or compound names differ, add a short explanatory note and the correct document.
• Justificante on paper: with the expediente number, the health centre can usually help you get attention before the physical card arrives.

Typical mistakes that can derail the procedure

1. Expired empadronamiento: they’ll reject it and you’ll have to start again.
2. Not proving the vínculo: without libro de familia or certificate there’s no beneficiary registration.
3. INSS right unresolved: the CCAA can’t give assistance if the INSS hasn’t recognised the right. Expect a block.
4. Uploading blurry photos: if the DNI/TIE isn’t legible they reject the file automatically.
5. Minor custody issues: without the custody ruling or the other parent’s authorisation, the expediente stalls.
6. Signing with the wrong browser: signature error and you lose the session. Switch browser before you hit submit.
