- 1.Book an appointment at your town hall's Servicios Sociales to open a file and get the municipal social report (usually mandatory).
- 2.Gather medical reports and request the certificado de empadronamiento histórico (covering at least 2 years) and the current empadronamiento from your ayuntamiento.
- 3.If you submit online, ID yourself with Cl@ve or a certificate in the Registro Electrónico Común (REC) and choose as recipient the consejería de Servicios Sociales of your CCAA: https://rec.redsara.es/registro/action/are/acceso.do
- 4.Fill in your CCAA's official application form (they give it to you at Servicios Sociales or on the regional electronic site) and submit it with all attachments via REC or at the CCAA's in-person registry.
- 5.Answer the call from the valoración unit for the home visit (baremo BVD/EVE). Have reports, medication and assistive devices ready; the person who cares for you should be present.
- 6.You receive the resolution of degree and are called for the PIA (Plan Individual de Atención): there you choose service or benefit. If it's an economic benefit, sign and give IBAN; if it's a service, they will process placement/home help.
