- 1.Go to the health portal of your comunidad autónoma and log in with Cl@ve or a digital certificate (e.g., Madrid: https://www.comunidad.madrid/servicios/salud/libre-eleccion-medico-enfermero-y-pediatra; Andalucía: https://www.sspa.juntadeandalucia.es/servicioandaluzdesalud/ciudadania/libre-eleccion; Catalunya: https://catsalut.gencat.cat/ca/ciutadania/tria-metge/).
- 2.Find “Libre elección” or “Cambio de médico/a de familia/enfermera/pediatra” and select your current centre or the new centre if you’re changing zone.
- 3.Check the available professionals (quota, morning/afternoon shift) and pick the one that suits you. If the quota is closed, the system will say it cannot accept more cards.
- 4.If you change centre or you’re outside your zone, upload the certificado de empadronamiento and, if requested, the proof of work/studies. Confirm the request.
- 5.You’ll get a confirmation in your health folder or by SMS/email. Assignment usually applies in 24–72 hours within the same centre; changes of centre can take longer.
- 6.If you can’t do it online, go to administración at your health centre with your DNI/NIE and TSI and ask for the change at the desk. They’ll process it and give you a receipt.
- 7.Download the updated tarjeta sanitaria digital if your CCAA offers it; if they issue a new physical TSI, they’ll tell you how to collect it or will send it by post.
