Request your Medical Record (autonomous CCAA and HCDSNS)
I explain how to view and download your medical record from your CCAA Carpeta de Salud and how to request a full copy from the hospital if you need it. Works for you, your minor children, or someone you represent (with the right paperwork).

What you need to have ready
- Cl@ve Permanente, certificado FNMT or DNIe. Cl@ve PIN sometimes only shows basic stuff.
- DNI/NIE and Tarjeta Sanitaria Individual (TSI) from your CCAA.
- If requesting for someone else: libro de familia or tutela, authorisation and the patient’s DNI when needed.
- Specify period and centres (e.g., “Hospital X, 2015–2020”). Saves you rounds.
Documentation
- Identification: DNI/NIE and strong authentication (Cl@ve Permanente/certificado/DNIe).
- Representation: minors (libro de familia), incapacitated people (resolución judicial), deceased (defunción + kinship/legitimate interest).
- Request for in-person or by registro: they give it at Admisión; online you can use the Registro Electrónico Común if your CCAA has no own procedure.
- Email/phone in case they need to clarify something.
Step by step
- Access your CCAA Carpeta de Salud with Cl@ve Permanente/certificado. Search “Historia clínica”, “Mis informes” or “HCDSNS” and download the PDFs already there.
- Check the HCDSNS: from your portal you’ll see reports from other CCAA (discharges, emergencies, consultations). General info here: https://www.sanidad.gob.es/profesionales/hcdsns/home.htm
- Missing documents (e.g., evolution notes, consents, old tests)? Request them from the hospital: in person at Admisión/Archivo or electronically via REC (https://rec.redsara.es) addressed to the centre or your Health Service. Write: “Ejercicio de derecho de acceso a historia clínica”, state period, centre and what you want.
- If you want images (RX/TC/RM), say so explicitly. They’ll give a download link or contact you to pick up a CD/USB with a DICOM viewer.
- Wait for their reply (usually 1–4 semanas) and check what they give you. If there are gaps, file an ampliación citing specific episodes.
Time-saving tricks
- Narrow the dates. Asking for “everything since 1990” usually blocks the process and slows you down.
- Cl@ve Permanente + Chrome: fewer signature/download issues than other browsers.
- If you lived in several CCAA, check your Carpeta and the HCDSNS. Regional + interoperable records cover a lot without waiting for the archive.
- Save every PDF as you download. Some portals drop your session and it’s hard to get the files again.
- If you changed TSI/address, request unification of your record at the health centre to avoid duplicates.
Common mistakes that can ruin the request
- Not proving representation (minors/deceased/incapacitated): instant denial.
- Asking for “everything” without details: they send basic reports and nothing of the evolution/consents.
- Relying only on SMS Cl@ve: limited access; use Permanente/certificado.
- Forgetting images: reports may be there, but the DICOM won’t; request it separately.
- Not using REC when there’s no procedure: you’ll be left waiting. The REC (https://rec.redsara.es) lets you file a legally valid request to your hospital/Health Service.
- 1.Log in to your CCAA Carpeta de Salud with Cl@ve Permanente, certificado or DNIe and look for 'Historia clínica', 'Mis informes' or similar (Catalonia: La Meva Salut; Madrid: Mi Carpeta de Salud; Andalucía: ClicSalud+…).
- 2.Check what’s available in the 'Historia Clínica Digital del SNS (HCDSNS)' from your regional portal (discharge reports, emergency notes, consultations, lab results). General info: https://www.sanidad.gob.es/profesionales/hcdsns/home.htm
- 3.If you need a full copy or documents not online (e.g., evolution notes, consents, etc.), file a request via the Registro Electrónico Común addressed to your hospital or your CCAA Health Service: https://rec.redsara.es (subject: 'Ejercicio de derecho de acceso a historia clínica', stating periods and centre).
- 4.If you prefer in person, go to Admisión/Atención al Ciudadano at your health centre or hospital with your DNI and fill their form requesting 'copia de historia clínica' and the specific period.
- 5.Download your reports as PDF from the Carpeta de Salud when published or wait for notification from your CCAA/centre. If you asked for imaging, they will give a download link or book you in to collect a CD/USB.
- 6.Check that everything is there. If episodes are missing or from another CCAA, open an incident in your portal or file a written request via REC to your Health Service specifying what is missing.
- DNI/NIE and Cl@ve Permanente, certificado digital FNMT or DNIe (for online access)
- Tarjeta Sanitaria Individual (TSI) or your CCAA sanitary number
- If acting for a minor: libro de familia or birth certificate and your DNI
- If acting for a legally incapacitated person: resolución judicial de tutela/guarda and your DNI
- If acting for a deceased person: certificado de defunción and a document proving legitimate interest/kinship (Ley 41/2002)
- Signed authorisation from the patient and a copy of their DNI if a third party acts
- If you go in person: the centre’s medical record access request form (they give it to you at Admisión)
Legal deadline
They have one month to give you access/copy under RGPD. What’s already in the Carpeta de Salud is immediate if uploaded; old paper records can take weeks.
Third-party access
For minors, incapacitated people or deceased patients they will ask for paperwork (libro de familia/tutela/defunción + kinship/interest). Without that, they’ll deny access.
Images and CDs
X‑rays/CT/MRI aren’t always in the Carpeta. They usually give a download link or a CD/USB with a DICOM viewer. The physical medium may carry a small fee.
Duplicate identities
If you changed TSI or lived in several CCAA, records can be duplicated or have gaps. Ask for unification of the record at your health centre before escalating.
Pesky tech
Some portals fail on certain browsers or signatures. Cl@ve Permanente + Chrome usually works better. Save each PDF; sessions can time out very fast.
What exactly is included in the online medical record?
Usually discharge reports, consultations, emergency notes, lab results and prescriptions. Medical images (DICOM) and internal evolution notes aren’t always available; you must request those from the hospital.
Can I see records from another autonomous community?
Yes, many reports are shared via the HCDSNS. It’s not perfect: some centres don’t upload everything or there are syncing delays.
Can I request my child’s record?
Yes. Up to 16 years, as parent/guardian with libro de familia. From 16, you need the minor’s consent except in specific clinical exceptions.
Can I request the record of a deceased relative?
Yes, but limited to what doesn’t affect third parties’ privacy. You must prove kinship and legitimate interest (Ley 41/2002) and provide the certificado de defunción.
Can they charge for the copy?
Access is free. They can charge for physical media (CD/USB) or paper copies if you request them. Online PDF downloads are free.
Do I need Cl@ve or a certificate?
For online viewing, yes: Cl@ve Permanente, certificado FNMT or DNIe. SMS Cl@ve often has reduced functionality. In person, DNI and the centre form are enough.
How long does it take?
What’s already in the Carpeta is immediate. A full hospital copy can take 1–4 semanas depending on the archive and the period you request.

Historial Clínico en el sistema nacional de salud ofrece cobertura en todo el SNS, aunque la gestión puede depender de tu CCAA de residencia. La tarjeta sanitaria individual facilita la portabilidad.
¿Historial Clínico requiere volante médico previo?
Determinados trámites sanitarios, incluido Historial Clínico cuando aplique, requieren derivación desde atención primaria. Solicita cita con tu médico de cabecera antes de iniciarlo.
¿Cómo recibo los resultados de Historial Clínico?
