Use this to tell the public health system where you want to be seen for specialties and scheduled surgery, instead of the 'assigned' hospital. It’s useful if you live in a CCAA with free choice (eg. Comunidad de Madrid) and want to change public hospital without making a mess.

- 1.Go to the Libre elección sanitaria page of Comunidad de Madrid: https://www.comunidad.madrid/servicios/salud/libre-eleccion-sanitaria and check which centres you can pick with the hospital finder: https://www.comunidad.madrid/servicios/salud/hospitales.
- 2.Book an appointment with your GP online (Cl@ve/DNIe) at https://www.comunidad.madrid/servicios/salud/cita-atencion-primaria so they can issue the referral to the hospital you choose.
- 3.At the appointment, tell them the exact hospital. They will issue the volante with that centre. Ask for a copy (paper or PDF in your Carpeta de Salud: https://www.comunidad.madrid/servicios/salud/carpeta-salud).
- 4.If you already had a referral/appointment at another hospital, contact the Unidad de Atención al Paciente of the hospital you want to go to (phone/email on its page in https://www.comunidad.madrid/servicios/salud/hospitales) and request the change, providing DNI, TSI and your volante.
- 5.Check that they give you an appointment at the new hospital. If they don't call within 7–10 working days, call the appointment number of the chosen hospital (it's on their page) with the referral number to hand.
