If you think a public hospital/health centre caused you harm because of a failure, you use this to ask your regional Servicio de Salud for compensation. For medical errors, hospital infections, big delays in diagnosis or treatment, etc.

- 1.Request your medical record. If you don't have it, download it from your regional health portal: Cataluña (https://lamevasalut.gencat.cat), Andalucía (https://www.sspa.juntadeandalucia.es/servicioandaluzdesalud/ciudadania/clicsalud), Madrid (https://www.comunidad.madrid/servicios/salud/carpeta-salud).
- 2.Write the claim letter. Address it to your Servicio de Salud or Consejería de Sanidad (not the hospital). Say what happened, the damage, the causal link and give an approximate amount you're asking for.
- 3.Log in with Cl@ve/certificado to the Registro Electrónico Común (REC): https://rec.redsara.es/registro/action/are/acceso.do and submit your claim. Recipient: Servicio de Salud or Consejería de Sanidad of your CCAA.
- 4.Attach PDFs: full historia clínica, reports, invoices and, if you have it, a medical expert report. Tick the boxes authorising data queries if the registry asks you to.
- 5.If you file on someone else's behalf, give powers via Apodera: https://apodera.redsara.es/apodera/ (Apoderamiento Apud Acta electrónico) and attach the receipt.
- 6.Keep the registration receipt (CSV or asiento number). Activate notifications on DEHú: https://dehu.redsara.es so you don't miss requests.
- 7.If they request something, reply within the deadline (usually 10 days). When the resolution arrives, read it carefully and decide whether to appeal.
