Request a Second Medical Opinion in the public health system (your CCAA)
If you’ve got a serious diagnosis and want another specialist in the public system to review it, this explains how to ask your autonomous health service for a second opinion. How to do it online with Cl@ve and what exact documents they’ll ask for.

- 1.Check that your case fits the pathologies accepted by your CCAA. For example, Madrid publishes requirements and covered pathologies here: https://www.comunidad.madrid/servicios/salud/segunda-opinion-medica
- 2.If your CCAA has an online procedure, open its “Segunda opinión médica” page and press Tramitar. In Madrid, identify with Cl@ve or certificado from the page above.
- 3.Fill in the request: your details, TSI number, centre and service treating you, diagnosis and proposed treatment. Attach the signed clinical report and relevant tests as readable PDFs.
- 4.If your CCAA doesn’t have its own form, file the request via the Registro Electrónico Común (REC): https://rec.redsara.es/registro/action/are/acceso.do Address the letter to your Consejería de Sanidad and clearly state “Solicitud de Segunda Opinión Médica”.
- 5.Sign and register. Download and keep the receipt with the registration number. If you use REC, select electronic notification.
- 6.Track the expediente: on your CCAA portal (if available) or, if you registered by REC, from Carpeta Ciudadana: https://carpetaciudadana.gob.es
- 7.They will assign a centre/specialist for the second opinion and notify you of the appointment or how to send extra information. The resolution usually arrives in 2–4 weeks depending on the CCAA and complexity.
