Step by step
- Download the form from your Servicio de Salud website (called “Registro de Instrucciones Previas”, “Voluntades Anticipadas” or “Voluntades Vitales Anticipadas”, depending on the CCAA).
- Fill in and sign the document. Choose modality: 2–3 testigos, notario or funcionario habilitado (the model/CCAA will say which are allowed).
- If you go with testigos, have them sign and bring their DNIs. If you use a notario, request a copia autorizada.
- Online registration: log into your CCAA register sede with Cl@ve/certificado, start the “Inscripción de Instrucciones Previas” procedure and attach the signed PDF, DNIs and, if applicable, the notarial deed.
- In-person registration: go to the authorised point with the document and witnesses or with the notarial deed. Some CCAA require ID in person.
- Get the proof of registration and keep it.
- After 1–4 semanas, check your health folder or ask at your health centre that it appears and has been uploaded to the Registro Nacional.
Time-savers
- Name a representative and leave their phone number: it avoids delays in emergencies.
- Be clear and concise (don’t write a novel). Use the options in the form.
- Upload legible PDFs and, if the sede allows, combine pages into one file.
- Share a copy with family and your GP: it speeds things up if there’s confusion.
- If possible, sign the PDF electronically to avoid extra trips.
Common mistakes that will mess things up
- Assuming the notario alone is enough: if you don’t register it, systems might not find it.
- Invalid witnesses (direct relatives or staff treating you): your inscription will be rejected.
- Forgetting the representative’s acceptance: this can block the registration.
- Uploading blurry photos: you’ll get a requirement and face weeks of delay.
- Not checking the upload to the Registro Nacional: on the day it matters, it may not be there.
- 1.Go to your Servicio de Salud website and download the form called “Registro de Instrucciones Previas/Voluntades Anticipadas” (some CCAA call it “Voluntades Vitales Anticipadas” or similar).
- 2.Decide how you’ll formalise it: sign with 2–3 testigos, sign before a notario, or sign before an authorised public official (rules depend on your CCAA). Fill the document, sign it and get the required signatures/acceptances.
- 3.Log in to your CCAA’s Registro de Instrucciones Previas electronic office with Cl@ve or certificado digital (https://clave.gob.es) and submit the inscription request attaching the document and the required DNIs.
- 4.If you prefer in person: go to the authorised points (registro sanitario/centro de salud/unidad de voluntades anticipadas of your CCAA) with the document and testigos or with the notarial deed.
- 5.Keep the proof of inscription the sede or registro gives you (receipt/registration code).
- 6.After 1–4 semanas, check your health folder/medical record to confirm the entry appears and that it has been uploaded to the Registro Nacional (ask at your health centre if you don’t see it).
- DNI/NIE o pasaporte en vigor (original y copia digital si lo haces online)
- Documento de Instrucciones Previas/Voluntades Anticipadas firmado (modelo oficial de tu CCAA)
- Si nombras representante: sus datos y aceptación expresa firmada, con copia de su DNI
- Si eliges testigos: 2–3 testigos mayores de edad con sus DNI y firmas (normalmente no pueden ser familiares directos ni personal sanitario que te atienda, según CCAA)
- Si lo haces ante notario: copia autorizada de la escritura de instrucciones previas
- Tarjeta sanitaria o número de asegurado de tu CCAA (para vincularlo a tu historia clínica)
- Para tramitación online: Cl@ve/certificado digital (https://clave.gob.es) y los PDFs firmados/escaneados con calidad legible
If you don't register it, the hospital might not see it
A notarial deed alone doesn't always show up in systems. The critical thing is INSCRIPCIÓN in your CCAA register, which later syncs to the Registro Nacional.
Watch the fine print on witnesses
The number and who can act as witness changes by CCAA. Direct relatives or staff treating you are usually excluded. If they don't meet the rules, your inscription will be rejected.
Notary: useful but not free
Registration is free, but the notario charges fees. If you want to avoid costs, use witnesses or an authorised official if your CCAA allows it.
Changes and revocation
You can change or cancel your directives anytime. The latest registered entry annuls the previous one. Keep your representative updated to avoid problems.
Delay in synchronization
It can take days between your CCAA and the Registro Nacional to sync. Carry a copy and give it to your representative in case of emergencies during that gap.
Can I do everything online?
In many CCAA yes: you download the form, sign (with witnesses or notarial deed), scan and submit it on the sede with Cl@ve. Others require in-person signing or ID. Check your Servicio de Salud website.
Do I need a notary?
No. In most CCAA witnesses or an authorised official are valid. A notary is optional and convenient, but it costs money and you still must register it.
How many witnesses do I need and who can they be?
Usually 2 or 3 adults. Direct family members and health staff treating you are typically excluded. Bring their DNIs and have them sign the document.
When is it valid and how long until it shows up?
It counts from the inscription in your CCAA register. It usually appears within 1–4 semanas, and the upload to the Registro Nacional can take a few more days.
Does it apply across Spain or only in my community?
It applies across Spain because your CCAA uploads it to the Registro Nacional. If you only have a notarial deed that isn't registered, it may not be found in emergencies.
Can I change my mind or cancel it?
Yes. Submit a new inscription or a revocation and the most recent one overrides the previous.
What if my family disagrees?
The document and your designated representative are binding. If it's not registered or it's unclear, disputes happen. That's why registering and naming a representative helps.
