Recognition of the right to Public Healthcare (INSS) and registration of beneficiaries

Retroactive registration: 60 days. After that, contributions are not recognised.

For you and your family to get a public health card, the INSS must first recognise your right. This is for people starting work, moving to Spain, adding beneficiaries (partner/children) or who don't see the right in Tu Seguridad Social.

What to have ready
  • Cl@ve, certificado digital o DNIe to log into Tu Seguridad Social.
  • DNI/NIE and, if you're foreign, pasaporte and TIE or certificado UE.
  • Empadronamiento up to date if you're not registered with the Seguridad Social or if you're adding beneficiaries.
  • Vida laboral or alta (if you work or are self-employed).
  • Libro de familia or pareja de hecho for beneficiaries, and proof of cohabitation/income if applicable.
  • S1 if you bring the right from another EU/AELC country.

  1. Identity: DNI or NIE (+ passport if applicable).
  2. Residence: TIE or certificado de registro UE (foreigners).
  3. Address: certificado de empadronamiento reciente.
  4. SS situation: alta/vida laboral, contract or RETA proof (if you're active).
  5. Beneficiaries: libro de familia, pareja de hecho, DNIs/NIEs, cohabitation and income proof if there's economic dependence.
  6. EU export: formulario S1.
Step by step
  1. Check if you already have the right: go to https://tu.seg-social.gob.es and look at Asistencia sanitaria. If you appear as insured and beneficiaries are listed, you're done.
  2. You don't appear or someone is missing: from https://sede.seg-social.gob.es submit the Solicitud de reconocimiento del derecho a la asistencia sanitaria attaching the documentation. With Cl@ve it's faster.
  3. No electronic means: book cita previa INSS at https://sede.seg-social.gob.es/wps/portal/sede/sede/Cita+Previa and hand the papers at the counter.
  4. EU cases: upload/submit the S1 and your empadronamiento. If you add beneficiaries, include their documents too.
  5. Check notifications in Tu Seguridad Social. If they ask for more, submit a subsanación from the same sede attaching what's missing.
  6. If granted: with the INSS resolution, go to the Servicio de Salud of your comunidad (health centre or regional sede) and request the Tarjeta Sanitaria Individual.
Time-savers
  • Check Tu Seguridad Social first: if the right already appears you avoid paperwork.
  • Scan everything to PDF in one file per document type (ID, empadronamiento, etc.). The sede handles that better.
  • Book appointments early and check frequently; cancellations pop up and disappear fast.
  • Update your mobile in TGSS (https://importass.seg-social.es) to get SMS and log in without certificado if needed.
Common mistakes that mess up the process
  • Mixing up INSS and the card: INSS recognises the right; the tarjeta is issued by the comunidad autónoma. Don't wait for it to arrive automatically.
  • Poorly documented beneficiaries: without a registered pareja de hecho and proof of cohabitation they'll reject it. Prepare those proofs well.
  • Expired empadronamiento: they can ask for a recent one. Get a new certificado if it's old.
  • Uploading blurry photos: send readable PDFs; otherwise they'll ask again and that adds weeks.
  • Submitting duplicate requests: that only slows things down. If there's already a file in process, wait or respond via subsanación.

  2. 2.If you don't appear with the right or someone is missing, submit the Solicitud de reconocimiento del derecho a la asistencia sanitaria on the Sede Electrónica del INSS (https://sede.seg-social.gob.es) with Cl@ve and attach the documents listed.
  3. 3.If you don't have Cl@ve/certificado, book a cita previa at INSS to submit it in person: https://sede.seg-social.gob.es/wps/portal/sede/sede/Cita+Previa (choose ‘INSS – Asistencia sanitaria/otros’).
  4. 4.If you export the right from the EU, upload or hand in the formulario S1 and your empadronamiento; if adding beneficiaries, attach libro de familia/pareja de hecho and any documentation proving economic dependence.
  5. 5.Watch your expediente: on https://tu.seg-social.gob.es check the status and INSS notifications; if they ask for subsanación, upload the missing items from the same sede.
  6. 6.With a favourable resolution from the INSS, go to the Servicio de Salud of your comunidad autónoma (health centre or regional website) to request the Tarjeta Sanitaria Individual. They will ask for the resolution and your DNI/NIE.
  7. 7.If you move or change work situation, update your details (address and phone) at TGSS via Importass: https://importass.seg-social.es to avoid missing notifications.
  • DNI o NIE en vigor (y pasaporte si eres extranjero no UE)
  • Certificado de empadronamiento reciente (si no estás en alta en la Seguridad Social o para incluir beneficiarios)
  • TIE o Certificado de Registro de Ciudadano de la UE (si eres extranjero)
  • Justificante de situación en la Seguridad Social: vida laboral o documento de alta en Régimen General/RETA (si trabajas o eres autónomo)
  • Libro de familia o certificado de matrimonio/pareja de hecho y DNIs/NIEs de los beneficiarios (si vas a incluirlos)
  • Acreditación de dependencia económica del beneficiario adulto (si aplica): certificado de convivencia y/o ingresos
  • Formulario S1 emitido por tu país (solo si exportas el derecho desde otro país de la UE/AELC)
  • Resolución de residencia o TIE por razones excepcionales (si tu caso no es el típico de alta laboral)

¿Qué necesito para completar Asistencia Sanitaria Nacional sin que me lo rechacen?

Asistencia Sanitaria Nacional es gratuito en INSS / TGSS y se puede presentar por cuenta propia. El punto que interrumpe el trámite es que hace falta Cl@ve activa, y si no está dada de alta el registro previo puede tardar días. Además, presentarse sin la documentación exacta obliga a pedir cita otra vez.
